CMS is pleased to announce the release of the new 30 CCW Chronic Conditions file. This file contains validated and updated versions of the current 27 CCW Chronic Conditions, plus three new ones: Urologic Cancer (Kidney, Renal Pelvis and Ureter), All Cause Pneumonia, and Parkinson's Disease and Secondary Parkinsonism.
