Purpose of this document
ResDAC has created an optional checklist to help you organize and track your documents for an amendment to a DUA request. This document is not required, but is a resource to help ensure that your request forms packet is complete when you submit it.
Who uses this document
All RIF requests will benefit from confirming that all necessary documents are included prior to submission to ResDAC. This checklist is specific to requests for amendments to an existing DUA. Make sure you use the correct checklist.
