The data in this column indicates whether the resident had a fever.

The data in this column indicates whether the resident had vomiting.

The data in this column indicates whether the resident had dehydration.

The data in this column indicates whether the resident had internal bleeding.

The data in this column indicates that the resident had none of the above problem conditions.

The data in this column indicates whether the resident had a fall at any time in the last month prior to admission/entry or reentry.

This column indicates the number of falls code without injury a patient has had since admission.

The data in this column indicates the number of falls that resulted in no injury -  no evidence of any injury is noted on physical assessment by the nurses or primary care clinician; no complaints of pain or injury by the resident; no change in behavior is noted after the fall, since admission/entry or reentry or the prior assessment (OBRA or Scheduled PPS) which ever is more recent.

This column indicates the number of falls code with injury (except major) a patient has had since admission.

The data in this column indicates the number of falls that resulted in injury (except major) - skin tears, abrasions, lacerations, superficial bruises, hematomas and sprains; or anyfall- related injury that causes the resident to complain of pain, since admission or prior assessment (OBRA or Scheduled PPS) which ever is more recent.

This column indicates the number of falls code with major injury falls a patient has had since admission.

The data in this column indicates the number of falls that resulted in major injury - bone fractures, joint dislocations, losed head injuries with alered consciousness, subdural hematoma, since admission or prior assessment (OBRA or Scheduled PPS) which ever is more recent.

The data in this column indicates whether or not the patient had major surgery during the 100 days prior to admission.

Indicates whether the resident had major surgery during the 100 days prior to admission.

Indicates whether the resident had a major surgical procedure during the prior inpatient hospital stay that requires active care during the SNF stay.

Knee Replacement - partial or total requiring SNF service.

Hip Replacement - partial or total requiring SNF service.
