Assigns the provider location identification numbers that identify the geographic locations where the provider performs services.

The provider location identification number uniquely identifies the geographic locations applicable to the provider for each CCW_APR_LINK_KEY. The same PRVDR_LCTN_IDs may be used by multiple states; therefore this ID must be used in conjunction with the CCW_APR_LINK_KEY.
Values: Alphanumeric character string (e.g., 1, 27, 00001, 5867),and value ‘000’ means this is all provider locations.
