The end date of the provider’s enrollment in the Medicaid and/or CHIP program with the state.

Comments

If PRVDR_ENROLMT_START_DT is not NULL and PRVDR_ENRLMT_END_DT is NULL, PRVDR_ENRLMT_END_DT will be equal to “31DEC9999” in the T-MSIS source data.

Values: Date (numeric, system dependent) e.g., 05DEC2015, 31DEC2299
