Date of death of the provider, if applicable. Variable is populated only for individual providers ; derived from the same monthly Base record as the most recent value of the provider entity type code (PRVDR_ENT_TYPE_CD) in the calendar year.
Values: Date (numeric, system dependent) e.g., 07NOV2015, or Null/missing
