QE Data Use Agreement (DUA) Signature Addendum

The purpose of this document is to add data custodians with access to individual level data (defined as cell sizes less than 11) to the Data Use Agreement (DUA) or to update contact information of individuals already listed on the DUA. Only one signature addendum is needed per organization that is accessing individual level data.
