Purpose of this document
The Federal Agency Data Sharing Supplement is used, as an addendum to the RIF Data Use Agreement, to request CMS approval to redisclose linked data sets for research purposes.
Who uses this document
Federal Agency requesting organizations who are wanting to link or share CMS data with non-CMS data for research purposes. This form is not required for all federal requesters.
