Some pharmacies share a relationship with a common organization. For each of these pharmacies, there is a relationship ID (RELATIONSHIP_ID) and type (RELATIONSHIP_TYPE).
Values: MMDDYYYY (e.g., 11/28/2015), or null/missing (relationship date n/a)
