For each navigation-eligible beneficiary who was eligible for navigation and who opted in for and received navigation (i.e., non-missing NC_NVGTN_CASE_ID), this field indicates the date the navigation case terminated. Toward the end of the model, CMS advised bridge organizations to try to provide at least 3 months of navigation to those who opted in for navigation services. The termination dates for any navigation cases still open when the bridge organization ended participation were set to the last day of the bridge organization’s participation (reference Appendix B in the AHC User Guide for start and end dates for each bridge organization).
